| To pick up your child from the slopes - or have a friend
do it for you, complete this form and give it to your child's
bus chaperone in the morning. For obvious safety reasons,
if we do not have this completed, signed form, we cannot
release your child to anyone. The person picking up your
child will need to present identification to the chaperone. |
