| To pick up your child from the slopes - or have a friend
do it for you, complete this form and give it to your child's
bus chaperone in the morning. For obvious safety reasons,
if we do not have this completed, signed form, we cannot
release your child to anyone. The adult picking up your
child will need to present photo identification at the Chaperone Hut at Summit Central. |

First complete and pay for your Bellevue Ski School registration: To qualify for the
discounted Student BIG S Season Pass, you must attach a copy of your 2010 Bellevue
Ski School registration confirmation sheet as proof of enrollment to your BIG S Season
Pass application.

Students need only purchase the L-T-D Pass. Note that Ski & Bag Check available for an additional $30.
|
IMPORTANT! Both Student AND Parent must sign the first page of the Big S Season Pass Application.

Mail completed application by DECEMBER 15, 2009 to The Summit at Snoqualmie or hand deliver to Summit West Season Pass office.
